What is in the bundle

Three papers, each opening on its own in your account. New questions are added every 15 days and appear in your account automatically, with an email to tell you.

Not sure yet? The $3 sample gives you a few questions from this bundle to keep. Case simulations are a separate product, Step 3 CCS cases ($60, lifetime).

Day 1 recalls

Day 1 tests the science and knowledge needed to practise without supervision: diagnosis, mechanisms and basic management across systems. On the real exam it is 232 questions in twelve 30-minute blocks (usmle.org, checked October 2026).

Topics covered
  • Diagnosis from history, examination and first results across all organ systems
  • Mechanisms of disease and drug action applied to adult patients
  • Microbiology, pharmacology and pathology in a clinical setting
  • Normal findings, screening and prevention
  • Biostatistics and epidemiology basics, interpreting a study

Day 2 recalls

Day 2 tests management: the next step, the right treatment, prognosis and follow-up, in office, emergency and inpatient settings. On the real exam it is 180 questions in nine 30-minute blocks, followed by 13 to 14 case simulations.

Topics covered

Ambulatory care

  • Screening and adult vaccines by age and risk
  • Hypertension, type 2 diabetes, lipids, heart failure and atrial fibrillation management
  • Asthma and COPD stepwise treatment, smoking cessation
  • Depression and anxiety in primary care, thyroid disease, arthritis, low back pain

Emergency and inpatient care

  • Chest pain, arrhythmias, stroke time windows
  • Sepsis, anaphylaxis, status epilepticus
  • DKA and hyperosmolar state, electrolyte emergencies
  • GI bleeding, the acute abdomen, ectopic pregnancy

Special populations

  • Pregnancy, children and adolescents, older adults, end-of-life care

Biostatistics, ethics and patient safety

Calculations, study abstracts and drug advertisements with two or three linked questions each, plus the ethics and safety questions that appear in every block. A worked solution comes with every calculation.

Topics covered

Reading research

  • Study design, population, outcome and comparison; whether the conclusion follows
  • Relative against absolute risk reduction, surrogate outcomes, misleading graphs, statistical against clinical significance

Calculations

  • Sensitivity, specificity, predictive values, likelihood ratios, pre-test and post-test probability
  • Relative risk, odds ratio, hazard ratio, number needed to treat and to harm
  • Confidence intervals, p-values, power, type I and type II error, choice of test, survival analysis

Ethics and safety

  • Capacity, consent and refusal, end-of-life decisions, confidentiality and reporting duties
  • Breaking bad news, shared decision-making, conflicts of interest, research ethics
  • Error disclosure, root cause analysis, handoffs, medication reconciliation

How the questions are written

  • In the exam’s format: a clinical case and one best answer, in the style of the free sample questions on usmle.org. Concepts repeat because Step 3 is built from a published content outline.
  • Current US guidelines. When a guideline changes we update the question and its review date, unlike anonymous files shared on Reddit and Telegram.
  • Answers marked: each question shows four options with the correct one in bold.
  • Named author and reviewer on every paper. We correct mistakes readers report.

How access works

  1. Choose 1, 3 or 6 months, or the $3 sample, and enter your email; your account is created at this step.
  2. Pay by card or crypto and the papers open at once. Bank transfers open when we confirm them.
  3. Read online on a phone, tablet or computer, on one device at a time. There is no PDF download, and each page carries your email and order number as a watermark.
